In order to process your request, we will need the following information:

●Parent/guardian'sname, address, and contact information
●Type of recordsrequested (transcripts, attendance records, disciplinary records, etc.)
●Reason for therequest
●Signature of theparent/guardian or authorized representative
●Any other relevantinformation
